Registration Form
NOTE to all Team Managers :
1) Participating teams should enroll with 15 players
minimum
(14 + 1
GK). A deposit of $1500 per team ($100 per player) is
due by July 14th , to hold your team's spot. The deposit is
non refundable.
The final payment is due July 21st.
The maximum number of teams will be 8 per
camp session.
2)
Refund Policy - If a camper is unable to attend camp, a
refund less a $150.00 service charge per player, will be
returned after the last camp session. Cancellations must
be made 72 hours prior to the beginning of your camp
session to be eligible for a refund.
3) Team
manager will complete the below registration form , with
their contact information and team information. The team
manager will then send the registration form to the
below address ALONG with the deposit or full
amount AND each players parent consent form. Mail at one
time to :
